§ 3173.10 Form 3160-5, Sundry Notices and Reports on Wells.
(a) The operator must submit a Form 3160-5, Sundry Notices and Reports on Wells (Sundry Notice) for the following: (1) Site facility diagrams (see § 3173.11); 43 CFR 3173.11 -- Site facility diagram. § 3173.11 Site facility diagram. (a) A site facility diagram is required for all facilities. A surface use plan of operations (which may be included in the same Sundry Notice as the request for approval of commingling and allocation) if new surface disturbance is proposed for the FMP and its associated facilities are located on BLM-managed land within the boundaries of the lease, units, and communitized areas from which production would be commingled.
